New Patient Registration
If your are resident within the designated practice area (see location map on the above tab), you can register with the practice. At busy times, we maybe unable to register new patients and you may be asked come back at a more suitable time.
If you would like to register with the practice then please collect a New Patient Pack from the Health Centre. You will be asked to complete an official NHS registration form for each person or family member registering. You will also be given a new patient medical questionnaire and an ethnicity form, which provides us with basic medical information for the period until your notes reach the practice.
You will be asked to make an initial appointment with the healthcare assistant which must be kept otherwise you will not be accepted as a patient.
It is our policy that patients register with the practice and not with an individual GP. Your medical card will have a doctor's name attached, but this is purely for administrative purposes. You may indicate your preference to see or speak to a particular doctor, but the practice cannot always accommodate these requests as specific doctors may not always be available.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
