Please sign up for Online Services as the prescription request form has now been removed from our website. Email requests are also no longer accepted. You will need to order your prescription via www.patient-services.co.uk or send your completed repeat slip to us by post, or hand it in to the surgery prescription box. Repeat Prescription requests are not accepted at Reception. Alternatively, you might prefer to telephone the 24-hour repeat prescription answer machine on 446818; please have the details of your medication to hand.
To register for this On Line service please complete an application form via the link below. If you are unable to print these and would like them posted to you please use the ‘contact us’ section of the website or call 621726. Please return the completed form to the surgery. Application forms must be signed by the patient. You will then be emailed log in details.
Due to patient confidentiality each patient registering is required to use their own email address. The service is currently only available to patients aged 16 and over.
PLEASE ALLOW AT LEAST 2 WORKING DAYS FOR YOUR REQUEST TO BE ACTIONED AND CHECK WITH YOUR PREFERRED PHARMACY HOW MUCH TIME THEY REQUIRE TO DISPENSE YOUR MEDICATION. SPECIAL REQUESTS MAY TAKE UP TO 5 DAYS
Early orders without valid reason will not be processed.
Medications supplied are monitored by computer and from time to time it will be necessary to call you into the surgery to review your treatment.
Repeat prescription requests are dealt with Monday to Friday during working hours and will be available from your nominated Pharmacy 48 working hours AFTER receipt of the order. Early orders without valid reason will not processed.
Many patients are now requesting medications which are not on current repeat prescriptions and these requests may take up to 5 days to process.
Additional requests for letters etc may also take up to 5 days.
